Payment & Services: I understand that the services provided by All Dogs Durham are filled on a first come, first serve basis upon receiving a registration form, terms agreement, and payment. If registration, terms, and payment are not received and confirmed by All Dogs Durham, my space in the class or on the calendar for a private session is not guaranteed. I understand that by agreeing to payment, I have committed to the full length of the training program selected. I understand that I am responsible for payment in full regardless of whether I choose to complete the training program.

Refund Policy:  If I decide to withdrawal from a training service, I understand that I am responsible for payment in full regardless of whether I choose to complete the training program. I understand that refunds may be granted on the basis of extenuating circumstances subject to the discretion of All Dogs Durham. If a refund is issued, the amount will be pro-rated based on the number of remaining sessions and all refunds are subject to a $25 administration fee.

Missed Class Policy: I understand, as a curriculum class student that make up classes are not offered but I will still receive the email write up from that class. If more help is needed outside of class, all students are welcome to purchase a private session with their instructor.

For rolling admission classes, All Dogs Durham requires written advance notice in case of absence. No call/ no show students will not be offered make up session options and those credits will be applied to the missed class.

Tardy Policy: I understand if I arrive ten or more minutes late for a group class or private training appointment, I will not be admitted to that session and will forfeit that appointment. I understand that, by arriving ten or more minutes after the start time of my class or appointment, I will be considered a no call, no show and will not be permitted a make-up session, refund, or credit for the missed session.

Vaccines: I understand that training of any type is an activity requiring my dog to be medically sound. My veterinarian has examined my dog and agrees that my dog is in condition to perform in training classes. My dog is free of any infectious disease and is vaccinated for distemper, parvovirus, and rabies (the rabies vaccination is only required for dogs over four months of age). My dog is also free of contagious internal or external parasitic problems and is free of any contagious respiratory problem or skin disorder. I can provide proof from my veterinarian upon request.

Dangerous Behavior in Group Class: I understand that group classes are not intended to correct severe behavior problems or aggression. For the safety of the instructors, dogs, and families in the class, any dog that demonstrates aggression in class will be asked to leave the class. I understand, if my dog has a history of aggression, or if I am unsure about my dog’s suitability for the group class environment, I will consult with an All Dogs Durham team member before registering for the group class.

Liability: All Dogs Durham will endeavor to create as safe an environment as possible for the training of my dog and will offer only sound, safe, and responsible training and training instructions. However, to the extent that All Dogs Durham is insured for any unintentional or negligent errors, omissions, or incorrect assertions, All Dogs Durham will be responsible for any such acts or omissions, but only to the extent of such insurance. I have been told by All Dogs Durham and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others, and consequently I am and will remain responsible for the actions of my dog at all times, and I hereby agree to indemnify and hold harmless All Dogs Durham of any and all claims of injury, expense, costs, or damages caused by my dog. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.

I hereby agree to abide by the rules and policies of All Dogs Durham training services as set forth in this contract. I understand that attendance of dog training classes is not without risk to myself, members of my family, guests who may attend, or to my dog. In consideration of, and as inducement to the acceptance of my application for training membership in training classes, I hereby agree to indemnify and hold harmless All Dogs Durham, its officers, directors, instructors, agents, employees and/or representatives of any and all claims, or claims by any member of my family, or accompanying guests of mine of injury, expense, costs or damages to myself, my dog or any handler sponsored by me both in class and out of class. In addition, I agree that I will defend and indemnify All Dogs Durham for any injury, expense, costs or damages to any dog handlers or dogs, whether sponsored by me or not, or to third parties arising out of my own actions or the actions of my dog. I have read the above-stated provisions, and agree to accept those responsibilities.

In the event of a medical emergency during participation in our training services we seek your permission to provide necessary emergency veterinary care promptly.

By agreeing to this release:

I authorize Provider to take/transport my pet(s) to a veterinary office of their choice. I understand that Provider is not responsible for the results of the veterinary treatment or the loss of my pet. All known medical and behavioral history of my pet(s) has been disclosed to Provider. I specifically represent that my pet(s) has/have not been exposed to rabies or distemper within thirty days immediately prior to care. I further warrant that my pet(s) is/are current on all vaccinations.

Should your pet require urgent medical attention during services, we will attempt to contact you or your designated emergency contact immediately.

Any costs related to emergency care will be, within reason, covered by All Dogs Durham business and liability insurance. Any remaining balances with be your responsibility.

This authorization remains valid throughout training with us.
